New Patients

Bring your child’s insurance card, a parent or guardian photo ID, a list of medications and allergies, and any available immunization records. Write down questions or concerns you’d like to discuss. 

Contact our team to confirm scheduling availability and participation with your specific insurance plan. We’ll provide instructions for completing registration before your child’s first visit. 

Download the Medical Records Release below and submit the completed form to your child’s previous practice. Ask them to send medical and immunization records to Desert Bloom Pediatrics using the instructions on the form
